Required2 photos 35x45mm, matte, white background, 80% face, non‑smiling, without glasses/head covering.
Signed Cover Letter
RequiredAddressed to The Embassy of Czech Republic with duration, purpose, expenses clause; include all applicants' details.
Minor NOC (if applicable)
If minor with one parent/alone: notarised NOC from other/both parents with passport copy.

Important Notes
- • Applicants with blank passport or no prior Schengen/US/UK may be asked for interview.
